The dealer replaced the plugs, which it was due for, and wound up replacing a coil pack to correct the problem. Yes when the surging and rough idle thing is happening that's when the alternator makes the sound. I did put a voltmeter across the battery and the voltage does drop but only by about .5v From 14.2 down to 13.7 or so then back up again as it surges. I was expecting worse. For now replacing the alternator again is the plan.
If the plug wires are bad you should see some arching to the cylinder head or fuel rail. If the car idles rough or cuts off at low RPM or idle, the cause could be a failing O 2 sensor. The O 2 sensor is out of the loop at full throttle and will not affect the way the car runs. A rough idle can also be caused by a problem with the air flow box.

Jan 17, 2014 · I have been experiencing some annoying rough/erratic idles on cold engine starts. RPMs are jumping between 400 and 1000 until the needle settles at 800 after 30-40sec (see video). Engine/exhaust sound like a tractor during these first seconds and engine seems to stall almost.

Your looking for the voltage to be below about 0.5v. If the voltage is higher than that examine all the negative cabling for problems. Preform the same test with the positive side. Place the red lead on the positive battery terminal (if possible dig it into the battery post, not the terminal). Place the black lead on the bad post of the alternator.
I also notice a rough idle when those lights are on. All of this happens once the car (and alternator) are warmed up (about 5-8 minutes after start up). For a while, they just flicker but then they just stay on. During this, the voltage is usually normal when revved but really low when idling or coasting to a light.

My underdrive performance pulley sucks the voltage down @ idle/red lights, but thats normal for underdrive pulleys. I might get rid of them soon. And anyone out there.. take autozone/murray/advanced auto/pepboys alternator test with a grain of salt.

Having a a major dilema with a rough idle / Multiple Cylinder Misfire on my 2001 Chevy S10. Its a V6 4.3L with 100K miles on it. The first sign of the problem was the service engine light would blink intermittently when driving on the highway or revving the gas. The blinking would stop after a few minutes and the light would go away.
Idle is worse with air conditioner on. Doesn't seem to have rough idle problems when fast idle is on. (e.g. when it's been sitting overnight). Misfires were recorded at highway speeds once, but mechanic was unable to reproduce it.

Mar 14, 2016 · A rough idle can be many things but the O 2 sensors are just reporting a bad exhaust fuel to air mix outside of expected values. The 1st O 2 sensor reports back to the ECU and tells if the Oxygen mix is incorrect ( car to rich or to lean) the 2nd O 2 sensor is just for EPA values.
